  • Passport

    To Change Gender on passport: 

    The amazing news is you can select whatever gender marker you like when applying (ds11) or renewing (ds82) passport, "no proof" reqiured. (It is by far the easiest offical id for transgener indiviuals to aquire!)


    To change Name on passport: 

    Fill out ds82 renewal application of passport, and  Proivide offical copy of court ordred name change, as well as updtaed ID.

  • Social Security Card

    To Change Name on Social Security Card: 


    Bring offical/certified copy of court ordred name change, and multiple forms of ID to your local SSA Office.


    To Change Gender in Social Security System: 

    All you need is either a passport showing desired gender marker, or a letter from doctor a that follows this template, also check out this policy from SSA.

  • Drivers License

    To Change Name On AL Driver Licence:

    *We suggest, if possible, circumventing AL's tedious process by finding a state (WA for example) with a much less burdensome procces.  Find an address to change DL address too (family memebers, friends, make a month long vacation out of it, etc), update name through those local process.*

    1. Fill out PS-12 / Name Change Form


    2. (Each county in Alabama has diffrent background check requirments, contact your local probate court to find out which form(s) you need) Most require this Background Check Form


    2.1. Contact Local PD or Sheriff  Dept and inquire about setting up an appointment for fingprining or recieving  Fingerprinting Cards. 


    2.2. Mail completed form to:

    "Alabama Law Enforcement Agency

    Criminal Records and Identification Unit

    ATTN: Background Checks

    P.O. Box 1511

    Montgomery, Alabama 36102-1511", 


    Wait to recieve official background check.


    3. Get both documents notarized.


    4. Gather, Bring & File Doucments at your local probate court  :

    -Name Change Form

    -Background Check

    -2 proofs of residence 

    -photo id

    -birth certificate

    (Fee, ranges $25-$135)


    5. Attend hearing.


    6. Update name with social security, 


    7.  Go to DMV with the Court Order & new Social Security Card to update license.


    To Change Gender On AL Drivers Licence:

    In order to update Gender Marker on AL Driver License you must present an updated/amended birth certificate.

    This process varies depending on your birth state. (click here for state specific information)


    Those born in Alabama :

    As per Policy Order 63 & Alabama Code § 22-9A-19, 22-9A-21, Regretfully Alabama requires a 'proof of sex change' to amend birth certificate.


    1 After changing name, obtain letter (template) from  'operating surgeon' and proof of name change


    2 Fill out HS-33


    3 Mail documents to : 

    Alabama Department of Public Health, Center for Health Statistics, P. O. Box 5625, Montgomery, Alabama 36103-5625


    Those born in Florida : 

    1 Get a note from a doctor that follows this (template). 


    2 Fill out DH 429 Birth Certificate Amendment form & DH 430 Amendment Affidavit 


    3 Get documents notarized 


    4 Mail All Following Documents to :


    Department of Health, Office of Vital Statistics, Attn: Correction unit 

    P.O box 210

    1217 Pearl Street

    Jacksonville FL, 32231-0042


    -Doctors Letter

    -DH429

    -Dh430

    -Photo ID 

    -Check/Fee

    -Name change if applicable 

    *I recommend doing rush options.

  • Birth Certificate

    Obtain a ceritified copy of name-change court order


    2 Fill out form HS-33


    3 Mail Documents (with applicaple fees) to :

    Montgomery, Alabama 36103-5625"

Who Are We?

We are a non-profit community run organization that is dedicated to providing resources and support to Alabama's Transgender residents, making sure the government is following the law, and trying to bridge gaps of un-understanding in the community.


Why Do We Exist?

ALTRA was founded after it's creator experienced a myriad of rights violations, and misinformation from local state entities, while trying to update her documents. Having spent time, money, and stress, fighting the state government to simply follow their own current policies, it became apparent that Alabama's  Transgender community was and is being targeted and stifled by a  web of misinformation and publicly unavailable polices.
